Join the Mission First Club Donate your prize back to the MS Society and we’ll provide you with a tax receipt for the value of the prize. You can make your fundraising dollars go even further and allow more of the MS Walk proceeds to go towards to the MS Society of Canada's mission. Our mission: To be a leader in finding a cure for MS and enabling people affected by MS to enhance their quality of life.
$10 000+	You are on the fundraising honour roll! Join fellow top fundraisers in the 2014 MS Walk of Champion and join the movement to end MS on an international level OR receive a $1000 in gift cards from the options below Bonus Prizes: You are eligible to receive the White Tech Shirt and Packable Jacket! and will be invited to attend the Celebration Dinner of Champions !
$5000+ Bonus Level: Invitation to Celebration Dinner of Champions	You are on the fundraising honour roll! MS Walk Prize Details

• Prizes will be awarded on the basis of pledges submitted. To be eligible for a prize your pledge money and prize order form MUST be received at the MS Society no later than May 28, 2013
• Prize requests are not automatic and must be submitted on or after MS Walk day
• Prizes are not cumulative, with the exception of the MS Walk T-shirt, the Elite Feet Club and bonus prizes, which are in addition to the gift cards
• All MS Walk prizes will be ready to be picked up at the Ottawa Chapter starting in July. Participants will be notified via email or phone.
• Please note that all apparel items come in men's sizes only
• Prizes are based on individual and not team totals

MS Walk Prize Order Form

Download the prize order form in Word or PDF . Please submit your prize form with your total funds raised by May 28 . Prize forms and money collected can be submitted at the MS Walk on April 28 or can be brought to the Ottawa Chapter at 1826 Woodward Dr.
