Assistive Devices Program, March of Dimes Canada

March of Dimes Canada provides financial assistance services to help consumers acquire appropriate equipment.

Eligibility Criteria:

  • Adults over the age of 19 who reside in Ontario
  • Must have an ongoing physical disability
  • Financial assistance is based on both physical and financial need To apply, applicants must:
  • Complete the Assistive Devices Application form
  • Receive an assessment from an Occupational Therapist, Physiotherapist OR Specialist
  • Supply a copy of the Ministry of Health ADP application and approval forms (if applicable)
  • Supply a written cost estimate from a vendor
  • Supply a copy of the Notice of Assessment & T1 General Tax Form for both the applicant and the applicant's spouse

The client MUST NOT order or purchase equipment prior to receiving funding approval.
