Health Care Connect

Health Care Connect is a program initiated by the Ministry of Health and Long-term Care which helps Ontarians without a family health care provider find one.

  • The program refers people without a regular family health care provider to physicians and nurse practitioners who are accepting new patients in their community. After registering with Health Care Connect, a nurse - called a Care Connector - will be assigned to help find a health care provider in your area.
  • You should have a valid OHIP card to be part of the program.
  • You will be referred to a family physician or a nurse practitioner in your community, once one is found. You are then responsible for calling to book an appointment.
